How to use it
- Copy the questions below into your RSVP or registration form, or print the sheet for a meeting.
- Send the caterer the counts per diet and every allergy with its exact ingredient and severity.
- Keep the completed sheet with the person who receives the food on the day.

- NP: no pork
- NB: no beef
- H: needs certified halal

Severity: mild discomfort, serious, or life-threatening (carries medication).

Why ask for the exact allergy ingredient?
"Seafood" or "nuts" can mean many things. The caterer needs the exact ingredient, such as shrimp or peanuts, to check each dish with the kitchen.
Does filling in this form make food allergen-safe?
No. It gives the caterer accurate information to check with the kitchen. In a shared kitchen nobody can promise food is free of an allergen.
What does 五辛 mean on the form?
Many vegetarians in Taiwan avoid five pungent plants: onion, garlic, scallion, chives and leek. Asking separately avoids the most common vegetarian mix-up.
